Spinal Outreach Service Health Questionnaire (SOS-HQ)
This tool helps identify new problems, alert to red flags and prompt preventive care activities. It can be used to develop a General Practice Management Plan and plan Team Care Arrangements. Covering the SOS-HQ in sections can facilitate a 12-month cycle of preventive health care, allowing systematic monitoring and increasing intensity/coordination of professional input for those identified with greatest need.
The SOS-HQ is designed to be completed in part by the patient, the practice nurse and the general practitioner for maximum efficiency. It uses lay language and is typical of what spinal specialists would ask in a patient consultation. It is divided into 12 sections, which can be covered in 4 separate consultations. You can also download the entire questionnaire.
