Integrated Diabetes Care in the Community: A Statewide Approach
This workshop brings together key stakeholders and partners to identify integrated approaches that deliver value-based care for people with diabetes in the community. The workshop informs a statewide diabetes initiative being developed to support a coordinated approach to the management of diabetes across care settings. It builds on previous work of Ministry of Health’s Integrated Care, the NSW Diabetes Taskforce and the Sydney Partnership for Health, Education, Research and Enterprise (SPHERE) Diabetes Integrated Care Working Group, as well as the significant work occurring in local health districts (LHDs) and primary health networks (PHNs). The workshop will explore key priority areas, including the patient journey and identifying gaps, to improve diabetes care in metropolitan and rural areas and how these are linked to evidence and outcomes.
Who should attend
People with lived experience of diabetes, clinicians and leads working in community based integrated diabetes services (across LHDs and PHNs), primary care (including General Practice), Aboriginal Community - Controlled Health Organisations, our translational research partners (including SPHERE, Sydney Health Partners, NSW Regional Health Partners), and non-Government Organisations.
