Genetic and Genomic Testing Consent Forms

New consent forms to be used when ordering genetic and/or genomic tests have been developed for use within NSW Health from 2017. The consent forms are accompanied by Information Booklets and a NSW Health Genetics Services Privacy Brochure to provide patients with information that clearly explains genetic and genomic testing so an informed consent is able to be facilitated.

What are the new genetic and genomic consent forms?

For health professionals within NSW Health, there are 8 new genetic consent forms available (listed below). These are accessed via Stream Solutions in a self-print function or printed versions in a pad of 100.  The product codes for ordering are:

  • NH700247 - Consent: Genetic/Genomic Testing (For patients 14 years and above - not for Guardianship Act purposes)
  • NH700248 - Consent: Genetic/Genomic Testing (For parent/guardian of patients less than 16 years)
  • NH700249 - Substitute Consent: Genetic/Genomic Testing Guardianship Act 1987 (For patients 16 years and above where consent is provided by a person responsible)
  • NH700250 – Consent: Diagnostic, Predictive and Genetic Carrier DNA Testing (For patients 14 years and above - not for Guardianship Act purposes)
  • NH700251 - Consent: Diagnostic Predictive and Genetic Carrier DNA Testing (For parent/guardian of patients less than 16 years)
  • NH700252 – Substitute Consent: Diagnostic Predictive and Genetic Carrier DNA Testing Guardianship Act 1987 (For patients 16 years and above where consent is provided by a person responsible)
  • NH700253 - Consent: DNA Testing for serious adult Neurogenetic Conditions (For patients 14 years and above - Not for Guardianship act purposes)
  • NH700254 – Substitute Consent: DNA Testing for Serious Adult Onset Neurogenetic Conditions Guardianship Act 1987 (For patients 16 years and above where consent is provided by a person responsible)

These are to both inform the process of consent for health professionals for whom this may be unfamiliar and provide patients with information that clearly explains genetic and genomic testing so an informed consent is able to be facilitated.

What is Stream Solutions (NSW Health employees only)?

Stream Solutions is the website used to house all NSW Health’s state-wide forms. There is a link to this site on every LHD/SHN intranet home page. How do you suggest changes be made to these resources (NSW Health employees only)?

The consent forms, information booklets and privacy brochure will undergo a formal evaluation in 12 months (end of 2018) and changed as necessary. Over the next 12 months as you use this new information please provide any feedback directly to sally.howard1@health.nsw.gov.au
